Bing Index Issues

For the past seven years I have had problems with Bing (and its predecessors). I am considering removing m entire site from the index and re-submitting it later. Many previous emails and posts have been answered with form responses that did not address my questions. (I do not have any issues with other search engines.)
Please let me explain the issues regarding www.hill-country-visitor.com. I'm not certain that removing old urls is the best solution, and I would greatly appreciate your help and advice.
 
1) From what I can tell, our home page www.hill-country-visitor.com is not included in the Bing index.
 
2) At one time our internal urls looked like this (one example):
We have changed the urls and installed 301 redirects to:
 
From what I can tell, Bing has both urls indexed creating a duplicate situation. FYI, there were never two pages...we simply changed the url for a single page of code. In other words, there were never two pages, just a url change.
