Join as The Elderlaw Firm, Well-Spring Solutions, Triad Health Network, AuthoraCare Collective & HealthTeam Advantage celebrate National Health Care Decisions Day, an initiative to encourage people to express their healthcare wishes through all the proper channels and for providers to respect those wishes. This special event will include presentations from Risa Hanau, LCSW, Clinical and Community Educator from AuthoraCare Collective; Dennis Toman, JD & Owner of The Elderlaw Firm; & Angela Lewis-Myers, LCSW at Triad Healthcare Network.

Attendees will also receive resources from The Conversation Project to share with patients and loved ones for end-of-life planning, as well as a certificate for a free one-on-one consultation with an attorney if they are someone we can help. Additionally, all participants will have the opportunity to get a free health care Power of Attorney which can be scanned directly into Cone Health systems at the event. A notary will be onsite, so be sure to bring your photo id.

Powerful Tools for Caregivers is a six-week educational series designed to provide family caregivers with practical tools to care for themselves while caring for a loved one. This program helps caregivers reduce stress, improve self-confidence, communicate feelings, balance their lives, increase their ability to make tough decisions and locate helpful resources.
