Tri-State woman’s workplace death could have been prevented, officials say

The automotive supplier now faces more than $300,000 in penalties.

FRANKLIN, Ohio (WXIX) - Investigators with the US Department of Labor think a Franklin company could have prevented a local worker from dying on the job in October 2023.

Investigators say a 26-year-old woman had been employed with Forvia, an automotive supplier, for about a year when she was putting cardboard under a machine that bends vehicle exhaust pipes and was crushed.

OSHA started looking into it and investigators say they found the company didn’t properly train employees.

Because of that, workers operating the machines were exposed to hazards, according to the US Department of Labor.

Investigators issued 10 citations and said they think the company could have prevented the death by having proper machine guarding in place.

Forvia now faces more than $300,000 in penalties.

The company now has 15 days to comply, request a conference with OSHA’s area director, or contest the findings before the OSHA commission.
