Driver charged with motor vehicle homicide in Polk County crash that killed passenger

A weekend crash claimed the life of a man in Polk County, and the driver is being held responsible.

SHELBY, Neb. (KOLN) - A weekend crash claimed the life of a man in Polk County, and the driver is being held responsible.

According to the Polk County Sheriff’s Office, deputies responded to a serious two-vehicle crash in Shelby, Nebraska on Saturday just after 7 a.m.

Shelby Fire & Rescue, Osceola Rescue and Rising City Rescue were called to help.

The investigation revealed a Ford Focus struck a semi tractor-trailer head-on. Four people were in the Ford Focus. Two people, including the driver, were treated for minor injuries and released.

One of the passengers, Jose Sierra Peralta, was life-flighted to Lincoln and died from his injuries.

Another passenger, Azucena Del Carmen Lumbi Peralta, was critically injured. They were identified as the owner of the vehicle and cited for no proof of insurance.

The driver of the Ford Focus, Pedaro Ruez Izaguirre, was arrested on suspicion of felony motor vehicle homicide, reckless driving and no operator’s permit.

The semi driver was not injured and was not cited.
