These disciplinary and licensing actions were announced recently by Pennsylvania’s licensing boards against professionals who live and work in the Lehigh Valley area. Cosmetology

Lehigh County

Brittney N. Conrad, of Bethlehem, Lehigh County, was assessed a $500 civil penalty based on practicing without a license. (05/10/18)

Montgomery County

Anna K. Delzingaro, license no. CO268369, of Hatboro, Montgomery County, was suspended for one (1) year, with such suspension immediately stayed in favor of no less than one (1) year of probation, ordered to pay a civil penalty of $500, and her license will be publicly reprimanded, because she has been convicted of any felony. Northampton County

Cali Nails, license no. CY196640, of Easton, Northampton County, was ordered to cease and desist in the possession and use of Methyl Methacrylate substances in the practice of nail technology; they were suspended for a period of one (1) year, such suspension to be immediately stayed in favor of probation for a period of one (1) year, subject to the Board’s terms and conditions; they were ordered to pay a civil penalty in the amount of $1,500; and a public reprimand was placed on their permanent disciplinary record with the Board, because they operated as a nail technology salon in a grossly incompetent and unethical manner through the possession of Methyl Methacrylate substances in the practice of nail technology. Lehigh County

Alfred George Khallouf, license no. DS035075, of Allentown, Lehigh County, was indefinitely suspended retroactive to July 31, 2017, such suspension to be immediately stayed in favor of probation for no less than three years for violating a previous agreement entered into with the Commonwealth. Montgomery County

Ryan Joo-Yeul Baek, license no. DS037069, of East Norriton, Montgomery County, had his license to practice as a dentist in the Commonwealth of Pennsylvania suspended for a period of thirty (30) days because he was found guilty of a misdemeanor involving moral turpitude. (07/20/18)

Jonathan William Theis, license no. DS038087, of Conshohocken, Montgomery County, had his license to practice as a dentist in the Commonwealth of Pennsylvania revoked, because he is unable to practice dentistry with reasonable skill and safety to patients by reason of illness, drunkenness, excessive use of controlled substances or as the result of any other mental or physical condition. Heather Elizabeth Clark, license no. DS031551L, of Glenside, Montgomery County, was levied a civil penalty of two thousand dollars and had a public reprimand placed on her record with the board, because she knowingly or negligently employed a dental hygienist whose license or certificate was not current or had been suspended or revoked. Northampton County

Carl Joseph Milano, license no. DS020803L, of Easton, Northampton County, was levied a civil penalty of ten thousand dollars and had a public reprimand placed on his permanent disciplinary record with the board because he administered anesthesia while his permit to do so was expired. (07/20/18)

Palmer J. Cotturo, license no. DS015065L and permit nos. DN015065A and DP000611, of Mount Bethel, Northampton County, permanently and voluntarily surrendered his license to practice dentistry because he engaged in unprofessional conduct. (09/14/18)

Funeral Directors

Lehigh County

Daniel K. O’Donnell, license nos. FD138830 and FS0156636, of Catasauqua, Lehigh County, was issued a $2,000 civil penalty and was indefinitely suspended stayed in favor of probation for no less than one year based upon having engaged in fraud or deceit in obtaining or attempting to obtain a license on two occasions and being convicted of a crime of moral turpitude. Bucks County

Amanda Herlihy, of Bensalem, Bucks County, was granted a license to practice and immediately placed on probation, based on crimes of moral turpitude. (02/28/18)

Montgomery County

Greg A. Bolles, license no. MSG009724, of Gilbertsville, Montgomery County, was suspended for no less than three years retroactive to January 29, 2018 for violating a consent agreement entered into with the Commonwealth. (04/13/18)

Medicine

Montgomery County

John R. Filip, license no. MD030793L, of Bryn Mawr, Montgomery County, had his license to practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. Mehdi Nikparvarfard, license no. MD424252, of Narberth, Montgomery County, was indefinitely and actively suspended, for no less than eighteen months, retroactive to September 15, 2017, because he was convicted of a felony (9/05/18)

Shane D Danielson, license no. MD424844, of Elkins Park, Montgomery County, was indefinitely suspended, with a minimum of one (1) year of active suspension, effective March 1, 2018, which was the start of the immediate temporary suspension of Respondent’s license, because he cannot practice as a medical physician and surgeon with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic, narcotic or other drugs or alcohol, or mental incompetence. Frederick D. Burton, license no. MD031220E, of Wynnewood, Montgomery County, was placed on an indefinite period of probation and assessed a $5,000 civil penalty, based upon being found guilty of two felonies and failing to complete 100 credit hours of continuing education. (05/31/18)

Berks County

Teresa Kahn, notary commission no. 1287971, of Sinking Spring, Berks County, is suspended for six months, stayed in favor of probation, and required to attend six hours of approved notary education and pay a civil penalty of $250, based on findings that she failed to know through personal knowledge or have satisfactory evidence that the person appearing before her was the person described in and who was executing the instrument and failed to be familiar with the duties of a notary public. Bucks County

Anya Vinokurov, notary commission no. 1171993, of Southampton, Bucks County, was revoked, based on findings she failed to place her notarial seal in a manner capable of photographic reproduction, failed to keep and maintain custody and control of an accurate chronological register of all official acts done by virtue of Respondent’s notary commission, failed to indicate each notarization separately, failed to be familiar with the duties of a notary public, and was not of sufficient ability to hold the office of notary public. Montgomery County

Anne P. Bergin, notary commission no. 1203080, of Conshohocken, Montgomery County, agreed to the suspension of her notary commission for twelve months, such suspension immediately stayed in favor of probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil penalty in the amount of $500 and probation assessment fees in the amount of $600 because she failed to require personal appearance during a notarization; failed to maintain an accurate notary register by failing to record the fee collected and character of the act; failed to be familiar with the duties and responsibilities of a notary public; and failed to be of good character, integrity, and ability. Amani Tamimi, notary commission no. 1257205, of East Stroudsburg, Monroe County, permanently and voluntarily surrendered her notary commission because she made a material misstatement or omission of fact on her notary public appointment and reappointment applications by failing to disclose her 2009 guilty plea; failed to maintain an accurate notary register by failing to record the character of the act, date of the instrument, amount of the fee collected, and date of the act for every notarization; failed to be familiar with the duties and responsibilities of a notary public; and failed to be of good character, integrity and ability. Miosoty E. Ramos-Bauza, license no. RN671890, of Allentown, Lehigh County, was indefinitely suspended for no less than three years with the first 12 months to be actively served and the remainder stayed in favor of probation based on her felony convictions. (03/29/18)

Megan Christner Hawkins, license no. RN671676, of Bethlehem, Lehigh County, was ordered to surrender her license, because she is unable to practice professional nursing with reasonable skill and safety by reason of mental or physical illness or condition or dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination. Jessica Mary McDade, license no. RN564495, of Hatboro, Montgomery County, was suspended for no less than three years retroactive to February 7, 2018, based on violating the terms of a previous Board order. (04/27/18)

Melissa Suzanne Whiting, license no. PN281945, of Green Lane, Montgomery County, was suspended for no less than three (3) years, with such suspension immediately stayed in favor of no less than three (3) years of probation, because she is addicted to hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination. Lisa Marie Grover, license no. RN508731L, of Hatfield, Montgomery County, was indefinitely suspended until such time as she has completed any criminal probation. After six months of active suspension, the remainder of Respondent’s active suspension will be stayed in favor of indefinite probation until such time as she has completed any criminal probation, and her license will be publicly reprimanded, because she was convicted, or entered a plea of guilty, or entered a plea of nolo contendere, or has been found guilty by a judge or jury, of a felony or a crime of moral turpitude, or has received probation without verdict, disposition in lieu of trial or an Accelerated Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, the United States or any other state, territory, possession or country and has been convicted of any felony. Morgan Nichole Natale, license no. PN280790, last known of Pottsville, Schuylkill County, was indefinitely suspended retroactive to February 9, 2017, and assessed costs of investigation of $525 based upon her being addicted to alcohol, or hallucinogenic, narcotic or other drugs which tend to impair judgment or coordination. Jan Neil Widerman, license no. OS004029L, of Holland, Bucks County, was suspended for no less than nine months retroactive to September 27, 2017, based on misdemeanor convictions under the Drug Act. (04/19/18)

Lawrence I. Miller, license no. OS009548L, of Lansdale, Montgomery County, was indefinitely suspended until he applies for and receives approval from the Board for reinstatement, under the terms and conditions set forth by the Board including completion of an Individualized Physician Clinical Competency Assessment Program approved by the Board, because he engaged in unprofessional conduct, including any departure from, or the failure to conform to, the standards of acceptable and prevailing osteopathic medical practice. Bucks County

Ryan Andrew Stout, license no. RP443133, of Perkasie, Bucks County, was indefinitely suspended for no less than three (3) years, such suspension to be immediately stayed in favor of no less than three (3) years of probation, unless that period of probation is extended or modified for cause by mutual agreement of the Respondent and the Professional Health Monitoring Program (“PHMP”), Disciplinary Monitoring Unit (“DMU”) case manager, subject to the terms and conditions of the Board, because he is unable to practice pharmacy by reason of physical or mental disease or disability. Bucks County

Gus B. Klausa, license no. RS290588, of Holland, Bucks County, voluntarily surrendered his license to practice as a real estate salesperson in the Commonwealth of Pennsylvania, because he failed to comply with the continuing education requirements to renew his Real Estate Salesperson license in that he was fourteen hours short of the minimum fourteen total hours of continuing education required for the renewal of his license; and failed to provide accurate and truthful information in connection with an application for examination, licensure, registration or approval. Lehigh County

Stephen N. Walden, license no. AB046343A, of Slatington, Lehigh County, was ordered to pay a civil penalty in the amount of $3,500 and an assessment fee of $726.68 towards costs of investigation; and he shall attend and successfully complete at least thirty (30) hours of remedial education on the topic of Real Estate Law at the Broker level subject to the terms and conditions of the Commission, because he distributed the escrow money while there was a dispute; he violated the law in that his conduct in filling in the escrow disbursement section of the Termination of Sale Agreement demonstrates bad faith, dishonesty, and untrustworthiness; and he failed to deal honestly and in good faith. Monica McFadden, license no. RS341298, of Bridgeport, Montgomery County, was granted a probationary license through completion of her criminal probation and payment of all costs, fines, and/or restitution. (10/27/17)

RS274433, of Bethlehem, Northampton County, was levied a civil penalty of $2,800, ordered to attend and successfully complete the missing hours of continuing education within forty-five days, and submit acceptable proof of successful completion of the continuing education course(s), because he failed to comply with the continuing education requirements to renew his Real Estate Salesperson license in that he was fourteen hours short of the minimum fourteen total hours of continuing education required for the renewal of his license. K Bobie Amankwatia, license no. PC004101, of Emmaus, Lehigh County, was ordered to permanently and voluntarily surrender her license, because she engaged in a dual or multiple relationship with a client, engaged in sexual intimacies with a current client, failed to respect the dignity and filed to promote the welfare of clients by engaging in sexual intimacies with a client, engaged in a sexual or romantic counselor-client interaction or relationship and engaged in immoral or unprofessional conduct. Montgomery County

Michael Aiden Fitzsimmons, license no. SW131596, of Ambler, Montgomery County, was indefinitely and actively suspended for a minimum of one year and shall cease and desist from practicing as a licensed social worker or representing himself as a Board licensee during active suspension because he engaged in a dual or multiple relationship with U.Y., by providing U.Y. a cell phone and/or discussing becoming U.Y.’s guardian or foster parent. MV211424, of Langhorne, Bucks County, was suspended for two years immediately stayed in favor of no less than two years of probation and assessed a $500 civil penalty, based on his pleading guilty to several misdemeanor and felony crimes of moral turpitude. Larry L. Clark, Jr., license no. MV246114, of Blakeslee, Monroe County, permanently and voluntarily surrendered his license to practice as a vehicle salesperson in the Commonwealth of Pennsylvania, because he on two separate occasions was convicted, or entered a plea of guilty, or entered a plea of nolo contendere in a court of competent jurisdiction in this or any other state or Federal jurisdiction of forgery, embezzlement, obtaining money under false pretenses, extortion, conspiracy to defraud, bribery, odometer tampering or any other crime involving moral turpitude while his current license is in force or within five years prior to the application for or issuance of a license; and made material misstatements on his Salespersons Initial License Application and on his Salesperson Change of Employer/Reactivating License Application by failing to disclose the two criminal counts.
