The film was a co-production between the film companies of Bavaria (situated in Geiselgasteig near Munich, where most scenes were shot) and Atalanta (situated in Berlin). Some scenes were shot at the German motorcycle company, Zündapp. The film's sets were designed by the art directors Max Seefelder and Erich Zander. It passed censorship on January 23, 1934, and was presented to the public on January 31, 1934, in Munich and on February 22, 1934, in Berlin.

When a young couple of travelers stop in a local tavern for resting, the fiancé vanishes and her fiancée seeks him and meets his spirit entering through the wall. She finds an entrance and finds that the stranger is actually Death, who is tired of bringing suffering to the world. She begs for the life of her beloved fiancé, and Death proposes her to save one of three lives that are in the end. If she succeeds, Death will bring her lover back to live. The lady becomes a woman in Persia, in Venice and in China, and in all situations she fails to save her respective lover. Death gives her one last chance, if she manages to find within one hour a person in the village that could give up living. When the local hospital is burning in fire, the young woman realizes the only way to stay with her lover.

That ordeal involves two unsolved murders and an attempted abduction. While the police initially suspect a man who is indeed a criminal as the murderer in these two instances, Francis believes the murderer is a Dr. Caligari, and his pawn, a sleepwalker named Cesare. Francis and Alan first spotted Dr. Caligari and Cesare as a carnival sideshow act. Francis' belief is that Dr. Caligari is using an unwitting Cesare to to commit these acts of murder. As Francis nears the conclusion of his story, details contained within are shown not to be as their appear on the surface.

In San Francisco, the sportsman Kay Hoog tells to the members of a club that he has found a message in a bottle with a map from a Harvard's missing professor telling about a treasure of an Inca lost civilization still alive. He decides to go to Peru to seek the gold. However, members of the secret criminal organization "The Spiders" leaded by Lio Sha break in Kay's mansion during the night and steal the map. Kay Hoog travels to Peru, where he retrieves his map and a document about the Diamond Ship from The Spiders. Later he saves the Priestess of the Sun Naela and brings her to San Francisco. However, The Spiders kill Naela and Kay Hoog promises revenge for the death of his love.
