MANSFIELD - The following restaurant inspections with critical violations were conducted by Richland Public Health between July 29 and 31.

El Charrito, 80 W. Hanley Road, Mansfield, July 29, follow-up inspection). Food contact surfaces not easily cleanable (critical repeat).

Subway 53308, 976 Ohio 97, Bellville, July 29. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). TCS foods not being cold held at the proper temperature. Recently prepared rotisserie chicken in the walk-in cooler was at 65 degrees-Fahrenheit. PIC voluntarily discarded three packages of chicken (critical, corrected during inspection). TCS foods not being cold held at the proper temperature. Observed the top layer of tomatoes in the front serving line at 47-48 degrees-Fahrenheit. PIC voluntarily discarded approximately 2-3 pounds of tomatoes. Bottom layer of tomatoes was at 42-43 degrees-Fahrenheit. PIC placed into walk-in cooler to cool down. Ensure pans are not overfilled so that product is maintained at 41 degrees-Fahrenheit or below (critical, corrected during inspection).

Bob Evans Restaurants LLC 485, 2100 W. Fourth St., Mansfield, July 29. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Food package(s) received in poor condition (critical, corrected during inspection). Food not properly protected from contamination by separation, packaging, and segregation. Observed products in McCall cooler/freezer unit and in walk-in freezer to be uncovered and not protected from contamination. Employee placed lid/cover on all products (critical repeat, corrected during inspection). Food not properly protected from contamination by separation, packaging, and segregation. Observed lid to ice bins in front area to not be closed to protect ice from contamination. PIC closed lid to both ice bins (critical, corrected during inspection). Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Raw shell eggs not maintained at 45 degrees-Fahrenheit or below (critical, corrected during inspection).

Hidden Hollow Camp, 5127 Possum Run Road, Bellville, July 29, standard and variance inspection. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Working containers of poisonous or toxic materials not properly labeled (critical repeat, corrected during inspection).

Keaton's At The Mall, F-C3 Richland Mall, Ontario, July 30, PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical). PIC unable to demonstrate knowledge by having critical violations (critical). Person in charge did not ensure their employees are properly trained in food safety (critical). Handwashing sink is being used for purposes other than handwashing  (critical repeat, corrected during inspection). Food equipment surfaces not cleaned at required frequency (critical). Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed products in Maxx Cold refrigerator to not be properly date marked. Some products contained a day sticker of Friday, however PIC did not know if that was the past Friday or upcoming Friday. PIC voluntarily discarded products that were not properly date marked: collard greens, macaroni and cheese, rice, wing sauce, potatoes, hot dogs (critical, corrected during inspection). Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed sliced tomatoes and cut lettuce in front cooler to not be properly date marked. PIC states product was sliced yesterday and placed proper date mark on products (critical, corrected during inspection). Presence of live insects. Observed multiple fruit flies/gnats congregating on/near the potato slicer. Discussed keeping area clean and free of food residue and/or contacting pest control operator for service (critical). Wastewater and sewage is conveyed through an unapproved system. Observed the floor drain to be overflowing sewage waste onto floor when handwashing sinks, prep sink or 3-comp sink were running. PIC called plumber who will be at facility the next morning for repairs (critical).

Olivesburg General Store, 4778 Ohio 545, Ashland, July 30. TCS foods not being cold held at the proper temperature (critical, corrected during inspection).

The Local at 97, 58 Ohio 97, Lexington, July 31. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection).

Two Cousins Pizza Co., 103 N. Main St., Mansfield, July 31. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection).

Subway 25770, 14 Mill Road, Bellville, July 31, follow-up inspection. Insufficient air gap between the flood rim and the water supply inlet (critical repeat).
