Evergreen Elm Inc. is celebrating 50 years of service to people with disabilities. Throughout the years Evergreen Elm has created a safe and caring environment for people with disabilities to live, thrive and belong through a variety of services.

The Supportive Living Program consists of two services, Mental Health Supportive Living and In-Home & Community. Participants with these services need support with the skills of community living such as daily living skills, medication monitoring, shopping, home maintenance, medical/dental appointments, etc.

The Residential Program provides 24-hour care in homes throughout the community. It is a group living environment that promotes family-style living, while encouraging community involvement.

The Unlicensed Residential is an apartment unit complex. It also provides 24-hour care, but each participant resides in their own apartment to maximize the participants’ independence.

Evergreen Elm plans to re-introduce its Life Share program to match an adult with intellectual disabilities to a host family or person in the community to live together as a family unit. In addition to providing a good home, the host family participates in supporting the person in cultivating self-care, social skills and communication skills. In so doing, the provider assists the individual in becoming an active member of the community.

Evergreen Elm’s scope of responsibilities includes counseling, crisis intervention, promotion of self, self-care skills, monitoring of medical status, treatments and coordinating with physician offices for care.
